• Single isolates for standalone investigations H2 Healthcare pathogens (single isolate) form (PDF, 360 KB)
• Submission of multiple isolates of the sample species which are part of a batch investigation and epidemiologically linked H1 Healthcare pathogens (multiple isolates) form (PDF, 370 KB)
  H1 - Healthcare pathogens cont. page (PDF, 391 KB) ONLY to be used with the H1 form (above) when submitting more than 4 samples for the same service
• Staphylococcus aureus, Streptococcus pyogenes, P. aeruginosa or B. pseudomallei Serodiagnosis H3 LHCAI Serodiagnosis referral form (PDF, 316 KB)

When filling in request forms, please ensure that you use the correct form for the service you are requesting, that you fill in all sections and provide all requested information. This will ensure that we can process your request efficiently.

We cannot guarantee that work on specimens submitted on any other request forms will not be held up while additional information is sought from you.
